This unique format presents experiences with hysteroscopy from well-known gynecologists of Switzerland, Germany, and Austria. Hysteroscopy: state of the art is not meant to be a comprehensive textbook of hysteroscopy but rather a collection of focused articles from experts in a rapidly evolving field. After a foreword by Professor Hans-Joachim Lindemann highlighting the history of hysteroscopy, the role of hysteroscopy in diverse clinical settings is discussed. Several of the articles present the authors’ most recent clinical studies. Topics covered include the use of hysteroscopy to evaluate postmenopausal bleeding, diagnosis of endometrial cancer, investigation of abnormal uterine bleeding in premenopausal patients, adenomyosis, and infertility. The authors advocate diagnostic hysteroscopy in all invasive work-ups for infertility, emphasizing that approximately 20% of hysteroscopy examinations in infertile patients will demonstrate some type of intrauterine anomaly. The chapter on diagnostic hysteroscopy for abnormal uterine bleeding in premenopausal patients presents data from the authors’ observational clinical study of outpatient diagnostic hysteroscopy. New indications are featured in this text, such as the role of hysteroscopy in tamoxifen-induced hyperplasia of the endometrium. The authors of this chapter performed hysteroscopically guided pathologic evaluation of the endometrium in patients taking tamoxifen. A thorough critique of the misleading use of the term hyperplasia in patients taking tamoxifen is presented, as it is not a glandular hyperplasia but rather a stromal hyperplasia that is identified. Preoperative and postoperative management in patients with hysteroscopic surgery is addressed as well as a comprehensive review of hysteroscopic resection of submucous myomas. Classification of submucous myomas is outlined. Several chapters pertain to endometrial ablative techniques and include balloon methods, punctual vaporizing, photodynamic, microwave, radiofrequency, and cryotherapy methods. An additional chapter is devoted to the hydrothermablation system (HydroThermAblator, BE Medical Systems, Hackensack, NJ) which coagulates the endometrium with simultaneous hysteroscopic control. The results of the pilot study are presented. A further chapter gives an overview of the most common procedures of operative hysteroscopy and outlines the combined risks, indications for surgery, types of complications, and criteria for safe procedures. The use of the Hysterotrainer, an in vitro simulator for hysteroscopy and falloposcopy, is presented. The text concludes with a consensus statement for diagnostic and operative hysteroscopy presented at the Joint Meeting of the Societies for Gynecologic Endoscopy of Switzerland, Germany, and Austria (October, 1999). This text is a compilation of 17 articles that provides up-to-date information in hysteroscopic indications and techniques. The book is well indexed, with recent references from both Europe and the United States. Tables and figures are well organized; however, few color images are provided.
