Small bowel bacterial overgrowth is associated with abdominal symptoms. The aim of the present study was to evaluated the role of small bowel bacterial overgrowth in suspected functional dyspepsia and determine the incidence of functional dyspepsia in our unit. Methods: Patients that fulfilled the Rome II criteria for functional dyspepsia (typical symptoms for the required time and negative: CBC, amylase, aminotransferase, abdominal ultrasound and upper endoscopy) underwent lactulose breath test for small bowel bacterial overgrowth diagnosis. Those with positive diagnosis were treated accordingly without any other intervention. The negative or symptomatic after treatment patients with history of food allergy went on exclusion/challenge diet. Patients were evaluated every two months for at least 6 months. Those that persisted symptomatic were considered functional dyspepsia. Results: From January to August 2002, twenty four patients (10 ± 2.32 years, 25% males) were studied. All were of low socioeconomic strata. The dyspepsia was ulcer-like in 79.1% and dismotility-like in 20.8% patients. Breath-hydrogen testing for small bowel bacterial overgrowth was positive in 21 patients (87.5%), 18 of them were asymptomatic after treatment (without any other therapeutic intervention) and have persisted like this all the extended 15 months follow up period. Food allergy was the diagnosis in four patients and only two were considered functional dyspepsia (8.33%). The incidence of functional dyspepsia was 3.26 per each 1000 evaluated patients in our unit. Conclusion: In similar populations small bowel bacterial overgrowth should be ruled out in suspected functional dyspepsia. The incidence of functional dyspepsia in our unit is lower than the reported in the literature." @default.
