- W1964048897 abstract "This study aims to assess whether the “white coat” effect in obese is associated to changes in cardiac structure, diastolic dysfunction, adverse metabolic profile and central adiposity distribution. For this, 34 obese patients were studied, 25 of whom classified as normotensive (NT) and 9 untreated white coat hypertensive (WCH). WCH was defined by mean daytime ambulatory SBP and DBP less than 135 and 90 mmHg respectively, in the presence of hypertension in at least two visits in the office (SBP ≥ 140 or DBP ≥ 90 mmHg). All patients underwent standard M-mode and two dimensional echocardiography and the left ventricular mass (LVM), calculated by Peen Convention, was adjusted by height (LVM/h). Measures of septum (SP), posterior wall (PW), relative SP and PW thickness and left ventricular diastolic diameter were also analysed. Diastolic function was assessed by E/A wave relation. Metabolic profile was determined by serum dosage of fasting glucose, cholesterol, triglycerides, HDL and uric acid. Waist and hip measurements and its relationship (W/H) were used as indexes of body fat distribution. Age, body mass index and ambulatory blood pressures were statistically comparable in WCH and NT. Comparison of cardiac parameters showed no significative differences in both groups, although there was a tendency in WCH toward higher values of LVM/h (89.4 ± 15.4 vs 83.6 ± 17.0) and lower E/A (1.25 ± 0.39 vs 1.45 ± 0.49). WCH had significative higher levels of uric acid (5.17 ± 0.98 vs 4.1 ± 0.94, p < 0.01) and triglycerides (147.0 ± 113.0 vs 111 ± 86.5, p < 0.05). Evaluation of fat distribution revealed a significative higher waist (111.3 ± 7.0 vs 101.8 ± 8.0, p < 0.005) and W/H ratio (0.94 ± 0.06 vs 0.89 ± 0.05, p < 0.05) for WCH. It was concluded that obese patients with WCH are more prone to metabolic disturbances, to have a central adiposity distribution and with a tendency to changes in cardiac parameters, pointing toward a higher risk profile when obesity is associated to the “white coat” phenomenon." @default.
