- W1964123170 abstract "S195 During CEA, attempts to detect cerebral ischemia and status of collateral circulation using EEG, TCD & SEP have produced equivocal results. Using a new dual-probe NIRS system of our own design, we monitored cerebral oxygenation via continuous measurements of oxyhemoglobin (Delta HbO2), deoxyhemoglobin (Delta Hb) and Delta TotalHb, in [micro sign]mol/L tissue. METHODS: Following IRB approval and informed consent, 8 patients were studied. Two NIRS probes were secured to the R & L forehead. Each probe consists of 4 laser diodes and 2 photodetectors. RESULTS: NIRS responses can be categorized into 2 types. After internal carotid artery (ICA) clamping, 5 patients showed no or minimal changes in Delta HbO (2), Delta Hb or Delta TotalHb. The other 3 patients (Figure 1) displayed a rapid decrease in Delta HbO2 and Delta TotalHb with a reciprocal increase in Delta Hb, indicating decreased cerebral oxygenation. Shunt insertion was accomplished in [similar]2-3 min. With the shunt in place, Delta HbO2 remains below baseline and Delta Hb above baseline, implying incomplete restoration of ICA flow. Contralateral side changes were of minor magnitude. Postoperatively, no patients experienced neurological deficits.Figure 1CONCLUSIONS: This and other studies, [1,2] support the concept that NIRS can detect changes in cerebral oxygenation and assess the adequacy of collateral circulation and shunt flow during CEA. On clamping ICA, decreased Delta HbO2 and Delta Total Hb and increased Delta Hb suggest an inadequate collateral circulation in some patients. Additional quantification and patient outcome studies are required before NIRS is accepted as a reliable monitor during CEA." @default.
