En effet, un tiers de la population mondiale est infecté par le Bacille de Koch (BK) dont plus de 8 millions de nouveaux cas de tuberculose chaque année. La tuberculose pulmonaire est la localisation la plus fréquente. Son diagnostic est parfois difficile et souvent établi avec un retard diagnostique à l’origine d’une dissémination de l’infection. Le diagnostic de l’infection tuberculeuse repose essentiellement sur les tests immunologiques représentés par l’intradermoréaction à la tuberculine et les tests de détection de l’interféron gamma. Le diagnostic de tuberculose pulmonaire est orienté par le contexte épidémiologique, les symptômes généraux et respiratoires traînants, ainsi que les données de la radiographie thoracique qui révèle souvent des lésions évocatrices, contrastant avec un examen pulmonaire souvent normal. Le tableau radioclinique peut être atypique chez les sujets aux âges extrêmes et chez les immunodéprimés. La confirmation de la tuberculose est bactériologique. Les méthodes bactériologiques classiques restent la référence. Les tests innovants utilisant la technique de biologie moléculaire ont permis d’améliorer le diagnostic de tuberculose en termes de sensibilité et surtout de rapidité. Ces techniques restent toutefois d’usage limité.Tuberculosis is a major public health problem worldwide. Indeed, a third of the world population is infected with Mycobacterium tuberculosis and more than 8 million new cases of tuberculosis each year. Pulmonary tuberculosis is the most common location. Its diagnosis is difficult and often established with a delay causing a spread of infection. The diagnosis of tuberculosis infection is mainly based on immunological tests represented by the tuberculin skin test and detection of gamma interferon, while the diagnosis of pulmonary tuberculosis is suspected on epidemiological context, lasting general and respiratory symptoms, contrasting usually with normal lung examination, and a chest radiography showing suggestive lesions. The radioclinical feature may be atypical in patients with extreme ages and in case of immunodeficiency. Confirmation of tuberculosis is bacteriological. Conventional bacteriological methods remain the reference. Innovative tests using the technique of molecular biology have improved the diagnosis of tuberculosis in terms of sensitivity and especially speed.
