- W1964914096 abstract "Advance care planning (ACP) is the process by which individuals can ensure that their preferences guide care during times of decisional incapacity. The utility of ACP in the context of a multi-racial Asian society, where different cultures have disparate beliefs with regards to death and dying is less well understood. We conducted a pre-implementation feasibility study for the role of advance care planning on 5 patient-caregiver dyads from a memory clinic in Singapore. Subjects were recruited if they had mild dementia as indicated by a global Clinical Dementia Rating of 0.5-1, no significant mood disorders or behavioral problems, and had an identified caregiver. Clinical information including demographic details, comorbidity burden (Charlson score), functional status (modified Barthel and Lawton's scales for basic and instrumental activities of daily living, ADLs) and caregiver burden (Zarit) were obtained. Patients and their caregivers were surveyed individually for their understanding of ACP, and prior experience of communicating their end-of-life treatment preferences to their loved ones. Subjects surveyed had a mean age of 69.8 years old and 60% were males. The majority (80%) were of Chinese ethnicity, and had mean educational level of 12.6 years . They were mostly independent in basic and instrumental ADLs {mean scores Barthel and Lawton = 99.0 (out of 100) and 15.8 (out of 23) respectively}, and had a low burden of comorbid conditions (Mean Charlson = 1.4). Caregivers, in which 60% were spouses, also did not record significant stress {mean Zarit= 13 (out of 120)}. Prior to ACP counselling, only 20% patients and 40% caregivers had heard of ACP. While 80% of patients and caregivers rated their health status as good and 80% had thought of their healthcare proxy, only 40% made future care plans. For patient-caregiver end-of-life communication, all surveyed agreed with the importance of communicating to each other about their illnesses. While none of the patients admitted to any barriers in communication, 20% of the caregivers indicated this difficulty. This study indicated the need for further education and dissemination of information on ACP among subjects with early dementia and their caregivers in the context of an Asian multi-racial country." @default.
