- W1965211360 abstract "EnglishWe describe a 20-year-old man presenting intraperitoneal metastases from medulloblastoma following surgical insertion of a spinoperitoneal shunt. Medulloblastomas are highly malignant primary tumours occurring predominantly in children with a high tendency to recurrence and seeding throughout the CSF pathways and blood. CSF seeding to the pericerebral and perispinal spaces is even more common and occurs earlier than spread through the blood and is present in up to 30% of cases at the time of diagnosis. Further CSF seeding is possible even ten years after tumour detection. A rare complication of CSF seeding is the direct transport of tumour cells to the lungs through ventriculo-atrial shunts or the development of peritoneal metastases from a ventriculoperitoneal or spinoperitoneal shunt as in our patient. The risk of metastases from brain tumours through shunt seeding has been a matter of controversy in the literature, but there is currently general consensus that this complication is rare. Diagnosis of metastasis through shunting should be confined to cases with exclusive, initial or prominent abdominal seeding. As in our patient, peritoneal metastases may also develop without other systemic lesions or tumour recurrence in the primary tumour site." @default.
