Yet, despite this, no urolithiasis-specific instrument with which to measure patients' QOL exists. QOL is an appropriate and important endpoint in urolithiasis. We developed the Wisconsin StoneQOL, a disease-specific instrument to assess the QOL of patients with urolithiasis. METHODS Patients and urology providers identified important concepts in focus groups and cognitive interviews. Patients (n=34) were largely recurrent calcium stone formers (M:F, 1.5; 50 ± 3.2 y); all were considered stable with respect to their stone disease. A preliminary instrument was created with 65 topics. Patients provided feedback about the frequency and importance of each item, allowing for item reduction and further refinement of the instrument. A 32-question instrument, using a Likert scale similar to other QOL and psychometric instruments, was drafted based on calculated impact factors of the preliminary tool. 100 patients from our clinic participated in testing, which allowed us to assess the instrument's internal consistency, reliability, respondent burden, and ability to detect change. RESULTS Salient issues related to urolithiasis symptoms and effects on patients' QOL were identified and formulated into a 2-page survey. Respondent burden is acceptable. Internal consistency of the instrument is high (Cronbach's α-coefficients >0.86 for all domains). Preliminary testing differentiates between acute vs. stable stone disease with lower QOL scores associated with disease exacerbation (p<0.0001 for total instrument score). Principal components analysis suggests further refinement of the instrument is possible. Testing of the instrument's discriminant and concurrent validity are ongoing.
