The author defends the defect-specific approach toanterior vaginal wall reconstruction and claims that thisapproach can “directly restore” damaged/altered anatomy,following the “simple rules of proper reconstructive surgicaltechniques”.I agree that such an approach is possible, feasible andsuccessful in the posterior compartment, but there is no proofthat this is the case for the anterior compartment. Nobody hasever credibly shown fascial defects of the anterior compart-ment in vivo, either by imaging or intra-operatively — andI’m sayingthisassomeonewhohasbeenlookingbyimagingand intra-operatively for over 20 years. What people havetakenforparavaginaldefectsarelevator defects, and that’sanentirely different matter: well documented by MR andultrasound, palpable by anyone with an educated indexfinger, and occasionally visible in the labour ward.In my view the author is propagating rumour andhearsay as regards the discredited concept of paravaginalrepair.
