- W1966317897 abstract "The purpose of this study was to examine what, if any, differences exist between non-compliant medical and psychiatric patients regarding sources of medical care, affordability of and access to medications, and reasons for non-compliance. Convenience sample of patients with medical or psychiatric complaints who presented to the emergency department (ED) of an inner-city, Level 1 trauma center (60,000 annual visits) and stated they were non-compliant with their medications were enrolled in this IRB-approved study. Inclusion criteria: Age ≥ 18, English-speaking, medically stable, able to give informed consent, non-compliant with medications. A 45-question survey consisting of questions from the validated National Health Access Survey along with a demographic profile were administered. Data was inputted into SPSS (Chicago, IL version 16) and the results of the survey were analyzed via chi-square. Of the 52 enrolled, there were no statically difference between the two groups in terms of demographics except those specifically mentioned. The number of medications that the patients took varied widely in each group. Twenty-six percent of medical patients were diagnosed in the ED with musculoskeletal problems, 14% with respiratory problems, and 14% with neurological problems. Thirty-one percent of medical and 41% of psychiatric patients went to the emergency department most often for health care. Seventy percent in both groups stated they did not get prescription medications because they could not afford it. Fifty-seven percent of medical and 53% of psychiatric patients stated they needed mental care but could not afford it. Forty-six percent of medical and 67% of psychiatric patients stated they needed follow-up care but could not afford it. Forty-nine percent of medical and 60% of psychiatric patients are very worried they will not be able to pay their medical bills. Forty-six percent of medical and 53% of psychiatric patients stated they skipped medication doses to save money. Forty percent of medical and 60% of psychiatric patients stated they took less medicine to save money. Sixty-three percent of medical and 67% of psychiatric patients did not ask their doctors for lower cost medications. Forty-nine percent of medical and 44% of psychiatric patients stated that their main reason for non-compliance to medications was due to inability to afford the cost of medications. Fourteen percent of medical and 25% of psychiatric patients stated they were unable to comply with their medication regimen because they ran out of refills. Seventeen percent of medical patients stated they forgot to take their medications. Nine percent of medical and 13% of psychiatric patients said that they did not get around/put off taking their medications. There were no significant difference between the two population in regards to their survey results. When comparing noncompliant medical and psychiatric patients, we found that there were no statistically significant difference between the two populations in regards to their answers to the National Health Access survey questions. In regard to accessibility/affordability of health care and medications, medical and psychiatric patients did not differ as much as one might expect. Inability to afford medications, lack of transportation, and running out of refills (many do not see a primary doctor so they have to return to the ED for more medications) amongst other reasons all contributed to non-compliant behavior in both groups with cost being the primary barrier." @default.
