- W1966598427 abstract "The aim of the present study was to examine the organ extraction and the splanchnic haemodynamic effects of octreotide in cinhoticpatients with portal hypertension. Thirteen cirrhotic patients had hepatic venous catheterisation performed. Hepatic venous pressure gradient (HVPG), indoc anine green (ICG) clearance and hepatic blood flow (HBF Y were determined in basal state and during 60 minutes of octreotiie-infusion (bolus 0.75 M/kg followed by 0.75 wlkgih). Bloodsamples were simultaneously drawn frun the artery and from liver and renal veins for measurement of octreotiieconcentration. The extraction-fraction of octreotide in the liver was 0.05*0.05 (mean&EM) and in the kidneys 0.16iO.09. Heoatic dearance ias estimated to 60*54 ml/min (n=l 1). The exiraction fraction ratio (E&E ) was 0.32*0.31. No correlations were found between liv&%mctiin parameters and renal extraction-fraction or liver-clearance. Octreotide had no effect on hepatic venous pressure gradient (HVPG) or wedged hepatic venous pressure (WHVP) although free hepatic venous pressure (FHVP) increased durfng octreotide infusion: 7~1 mmHg vs. &l mmHg (p=O.O18). No effect on hepatic blood flow (HBF) was observed while ICG-clearance decreased significantly. In conclusion octreotide is extracted in cirrhotic patients by both the liver and the kidney, the latter being the most important organ of elimination. Octreotide decreases liver metabolic activity determined as ICG clearance, but no significant effects of octreotide on HVPG or HBF could be demonstrated.
