- W1966743516 abstract "During the last years linear array EUS has become more and more operative, especially in the oncological field. A new flexible bipolar hybrid cryotherm probe (CTP) (ERBE Elektromedizin GmbH, Tübingen, Germany) has been applied with success to the porcine pancreas. Feasibility, efficacy, and safety of its application to the liver and spleen were evaluated in a living pig model. The CTP combines bipolar RF-ablation with cryotechnology. The more effective cooling by cryogenic gas (Joule Thompson effect) increases the RF-induced interstitial devitalization. The probe has a diameter of 1.8 mm and can be safely passed through the operative channel of the echoendoscope. A Hitachi EUB 8500 ultrasonographic system (Hitachi GmbH, Hamburg) and a linear array Pentax EG3830UT echoendoscope (Pentax Precision Instruments Corp., Hamburg) with a 3.8 mm operative channel were used for the application of the CTP. The ablation was performed in a transgastric approach, after the application of Doppler to avoid interposing vessels between the stomach and the target organs.|Ten applications to the liver and nine to the spleen were performed in 19 pigs. Power input (16-18W) and simultaneous cooling with CO2 (675 psi) as cryogenic agent were standardized. Application time varied from 120 - 900 sec. The ablation area was measured by EUS right after ablation (T0), and before euthanasia (T1). Gross pathology (T2) and histology after necropsy served as gold standard. The interval between treatment and euthanasia was one or two weeks. Correlation of lesion areas T1 and lesion areas T2 was calculated by linear least squares regression. The relationship strength was evaluated by means of R index and relative p-values. Comparisons between lesion areas was performed by a Wilcoxon matched pairs test. Effective pairing was determined by Spearman correlation (rs index) and relative p-values. The correlation between EUS and gross pathology was quite good, but EUS tended to overestimate the area of the ablated zone (correlation coefficient Rliver = 0.71; Rspleen = 0.73). EUS was able to observe a time dependent ablation area: we demonstrated a positive trend of lesion size (T1) over time of application in liver tissue." @default.
