There is no case for preoperatively investigating patients with a history of allergy, atopy, or asthma, but there is a case for delaying surgery and investigating patients with a history of a previous reaction. Particular care should be taken in using cephalosporins in patients with penicilin allergy and specific enquiries should be made regarding allergy to latex. Most successful litigation is related to the quality of treatment and it is mandatory to give oxygen, secure the airway, give a sympathomimetic drug early (adrenaline is the preferred drug) and to give intravenous fluids. Mast cell tryptase level is the best test to determine whether or not an anaphylacoid or anaphylactic reaction has occurred and may be performed at postmortem. Follow-up testing including a skin test and adequate communication of the results are mandatory and is the responsibility of the anaesthetist who was in charge when the reaction occurred." @default.
