- W1968045430 abstract "Objective: The accurate preoperative diagnosis of fallopian tube malignancy continues to be one of the great challenges for sonologists as the incidence of the disease is low (3.6/1000000 women) and a high degree of expertise is needed for its detection. The purpose of this study is to illustrate the sonographic features of primary malignant tumors of the fallopian tube and to heighten awareness of their existence. Methods: This is a retrospective review of the imaging and clinical files of 4 patients with primary malignant tumors of the fallopian tubes found in the medical records of 4500 patients with primary gynecological malignancies diagnosed during the last 6 years in our institution. Three tumors were primary adenocarcinoma and one a malignant mixed Mullerian tumor. All patients (age range 41–67 years old) had transvaginal sonography prior to surgery. Images were reviewed to determine common features of this rarely encountered pathology. Results: Transvaginal sonography showed normal ovaries in three patients in association with a solid adnexal mass, varying in size from 1 to 12.5 cm. The origin of the mass from the fallopian tube was shown directly in 1 case where a large vascular solid mass was seen within the lumen of a dilated fluid filled fallopian tube. Localization to the tube was inferred in two further cases when a mass was seen in the expected location of the fallopian tube between the lateral margin of the cornua of the uterus and the ovary. Both of these masses were noted to be very mobile on real-time examination. Large solid adnexal masses of unknown origin were seen in one case. Additional findings include metastatic retroperitoneal adenopathy (n=1) and a small amount of free pelvic fluid (n=2). All patients had unilateral disease. Conclusions: A solid papillary intratubal mass allows for easy prediction of fallopian tube cancer but was not the major observation in our patients. These tumors should be considered when unexplained solid masses corresponding with the expected location of the fallopian tubes are seen in association with normal ovaries. Mobility of a pelvic mass is an additional feature, which should suggest the possibility of this diagnosis." @default.
