Currently it is recommended that all internationally adopted children be screened for hepatitis A, B and C and vaccinated against hepatitis B. We report a case of a false positive hepatitis B surface antigen test in an internationally adopted child within 2 h of vaccination with Energix-B. A 1-year old was seen in clinic 2.5 weeks after she was adopted from Russia. At the time the only reported illness was sinusitis treated 1 week earlier by her primary care physician. The child had vaccine records from Russia, indicating vaccination against diphtheria, pertussis, tetanus, polio, measles, mumps and rubella. With the parents’ consent, the child’s vaccinations were restarted, according to American Academy of Pediatrics Red Book recommendations for the vaccination of a child <7 years of age who had received no vaccines. 1 These recommendations call for the first diphtheria-tetanus toxoids-acellular pertussis, first Haemophilus influenzae type b, measles-mumps-rubella (if older than 12 months) and hepatitis B vaccines initially. The patient received the vaccines (including Energix-B) and subsequently was sent to the laboratory for phlebotomy. The time elapsed between receiving vaccinations and phlebotomy was ∼90 min. Hepatitis B serologic tests were negative for hepatitis B surface and core antibody but positive for hepatitis B surface antigen. This constellation of results generally implies a very recent or chronic infection with hepatitis B. Six weeks later hepatitis testing was repeated before the second set of vaccinations. The hepatitis B surface antigen and core antibody were negative, but the hepatitis B surface antibody was positive, consistent with receipt of hepatitis B vaccine. Transient hepatitis B surface antigen positivity after vaccination has been reported previously in the literature. One study found a rate of 50% transient hepatitis B surface antigen positivity lasting for 2 weeks or less. 2 In most other studies hepatitis B serologies were not obtained until 48 to 72 h after vaccination. 3 As the number of internationally adopted children continues to grow, the scenario we describe may become more common. Most parents of recently adopted children are anxious about the possibility of any illness, especially hepatitis B and HIV.
