- W1969366929 abstract "The aim of this study was to investigate the incidence of sonographic lean and large UCs in monochorionic and dichorionic twin pregnancies. The sonographic cross-sectional area of the UC was measured in all cases. The measurements were performed at three different parts of a free floating loop of the umbilical cord and the average of three measurements was used for statistical purposes. Lean and large UCs were defined as a cross-sectional area < 10th and > 90th centile for gestational age (GA), respectively. Pregnancies were classified as monochorionic or dichorionic based on typical sonographic criteria. IUGR and SGA was defined as abdominal circumference at sonography < 5th centile and birth weight < 10th centile, respectively. Birth weight discordance was defined as weight difference > 20%. During the study period 170 twin pregnancies were included. Median (range) GA at inclusion was 26 (18–38.2) weeks. Of these, 72 (35.8%) and 98 (64.2%) were monochorionic and dichorionic twins, respectively. The prevalence of lean UCs was similar in the two groups (25.8% vs. 24.7%). Large UCs were significantly more frequent in monochorionic than in dichorionic twins (27.4% vs. 13.2%; p < 0.01). This difference did not remain significant excluding cases with twin-to-twin transfusion syndrome (TTTS) (n = 20) from the monochorionic group. The incidences of lean and large UCs were significantly higher in monochorionic twins with TTTS than both, monochorionic twins without TTTS and dichorionic twins, respectively (lean: 45% vs. 16.7% vs. 24.7% and large: 42.5% vs. 20.2% vs. 13.2%; p < 0.01 between groups). No difference was observed in the incidence of IUGR, SGA, or birth discordance between groups. Compared to singleton pregnancies, a higher incidence of lean and large UCs has been observed in twins.
