However, anterior laparoscopic adrenalectomy requires opening the peritoneal cavity and mobilizing adjacent organs, both of which can be challenging in patients who are obese or have had prior abdominal surgery. The anterior laparoscopic approach also uses intraperitoneal insufflation pressures of 12 to15 mm Hg, which some patients will tolerate poorly from a hemodynamic or respiratory perspective. In addition, laparoscopic adrenalectomy usually is performed with patients in a lateral position; therefore, bilateral adrenalectomy performed laparoscopically is cumbersome, because it requires repositioning the patient midway through the procedure." @default.
