- W1969916797 abstract "Post-CABG cognitive decline often resolves in the short term, but the long term cognitive effects remain controversial. Older age and presence of other cardiovascular risks, such as diabetes mellitus (DM), appear to worsen long-term cognitive outcome in these patients. The aim of this study was to determine the relationship of CABG to cognitive status and MRI findings in the Florida Alzheimer's Disease Research Center (FADRC) cohort. Subjects in the FADRC represent a convenience sample of participants over 65 years of age (mean = 74±6 yrs). All subjects received a complete dementia work-up, including a brain MRI, which was graded for periventricular and centrum semiovale white matter hyperintensities (PV-WMH and CS-WMH) and medial temporal atrophy (MTA), using a computer-assisted method to improve reliability and sensitivity. Episodic memory was assessed using the Fuld Object Memory Evaluation (FOME). A medical history, including a past history of CABG, was obtained for subjects classified as no cognitive impairment (NCI; n=228), non-amnestic mild cognitive impairment (naMCI; n=106), amnestic MCI (aMCI; n=133) or Dementia (DEM; n=112). The frequency of CABG among NCI (4%), naMCI (4%), aMCI (13%) and DEM (12%) was associated with severity of cognitive diagnosis (χ2=16.2, p=.0009), even after controlling for age, education and other cardiovascular risk factors (e.g, hypertension, diabetes and hypercholesterolemia) (p<.05). Among aMCI and DEM subjects, a history of CABG was associated with greater PV-WMHs (1.5±1.1 versus 1.0±0.8; p<. 05) and CS-WMHs (2.1±1.4 versus 1.1±1.2; p<. 05). Subjects with a history of CABG and with WMH ratings above the 50th percentile had higher MTA scores (p = .001), but did not have significantly lower FOME scores when compared to those with WMH ratings below the 50th percentile. A history of CABG is an apparent risk factor for developing aMCI and dementia as well as for having higher WMH ratings. Those with CABG who had high WMH ratings had greater medial temporal atrophy, suggesting that there are related mechanisms for developing WMH and neurodegenerative disease among these subjects." @default.
