- W1970528591 abstract "Synopsis: Hypoglycemia is a common complication in hospitalized patients. Affected patients tend to have an increase in cost and duration of their stay. Previous research revealed that these effects are markedly stronger in nondiabetics than in diabetics. Current data outline multiple risk factors for hypoglycemia in diabetics. However, to date there is only limited information concerning risk factors in nondiabetics. Purpose: We sought to determine 1) whether nondiabetics with hypoglycemia have a risk factor profile that is distinct from nondiabetics without hypoglycemia? 2) Whether their risk factor profile differs from than that of diabetics with hypoglycemia? 3) Whether there are differences in morbidity and mortality between the groups. Methods: We conducted a retrospective chart review of all adult patients who experienced a hypoglycemic episode, defined as a blood glucose of less than 40 mg/dL on any point-of-care or laboratory examination. Nondiabetics who did not have any hypoglycemic episodes during their hospitalization, matched by age and admission diagnosis, were chosen as controls. Data regarding known risk factors for hypoglycemia (including hepatic and renal failure, malnutrition, heart failure and sepsis) as well as outcomes were collected, and the groups were compared by use of the chi-square test. Results: In studying the nondiabetic groups, there was a significantly greater prevalence of sepsis and malnutrition in those with hypoglycemia compared with those without (P = .000). Interestingly, there was no significant difference between the two groups in the use of medications associated with hypoglycemia, including insulin and oral hypoglycemics (P = .54). Patients with hypoglycemia were more likely to spend time in an intensive care setting (P = .000), had a significantly increased mortality (P = .000) and a significantly higher risk of requiring placement in a care facility after discharge (P = .026). Second, we compared the nondiabetics with hypoglycemia to diabetics with hypoglycemia. There was no difference in the presence of any of the studied risk factors, or the need for placement. However, nondiabetics were more likely to spend time in an intensive care setting and had a significantly greater mortality than diabetics who experienced hypoglycemia (P = .000). Conclusions: The results of this study suggest that hypoglycemia may be useful as a marker of increased morbidity and mortality in hospitalized nondiabetics. Considering the different risk factor profiles of nondiabetics with and without hypoglycemia, it might be pertinent to search for an underlying condition if a patient experiences a hypoglycemic episode rather than simply providing symptomatic treatment. It will be interesting to conduct further studies investigating this hypothesis." @default.
