- W1970748897 abstract "The Nuclear Regulatory Commission (NRC) regulations for the release of patients following administration of radioactive materials states that patients can be released without instructions if the total effective dose equivalent (TEDE) to the maximally exposed person is less than 1 mSv (100 mrem). This study's purpose is to estimate the maximum possible radiation dose to other individuals from patients treated with Y90 hepatic radioembolization, to identify patient/procedural factors contributing to higher contact TEDE and to determine if it is necessary to provide patients with instructions on how to minimize dose to contacts. Dosimetry data was analyzed after 143 consecutive administrations of Y90 (124 SIR-Spheres, 19 Theraspheres) in 86 patients. Patient dose rates at skin level, 1 foot and at 1 meter were measured immediately after Y90 infusion. TEDE to the maximally exposed individual was calculated based on the total-body residence time and measured dose rate for Y90 microspheres. This was compared to the NRC release criteria. Patient/procedure specific factors studied included the treatment volume and location, lung shunt fraction on MAA study and body mass index (BMI). The mean administered activity was 0.98 ± 0.94 GBq (26.6 ± 25.3 mCi) (SIR-Spheres 0.71 ± 0.35 GBq [19.2 ± 9.4 mCi], Theraspheres 2.75 ± 1.52 GBq [74.3 ± 41.1 mCi]) (range, 0.07–5.14 GBq [1.9–139 mCi]). Immediately after treatment, the mean dose rate at 1 m was 1.2 ± 1.3 μSv/h (0.12 ± 0.13 mrem/h) (range, 0.02 - 9 μSv/h [0.002–0.9 mrem/h]). The mean TEDE to the maximally exposed contact was 0.02 mSv (2.8 mrem) (SIR-Spheres 0.02 mSv [2.4 mrem], Theraspheres 0.05 mSv [5.2 mrem]) (range, 0.0004–0.22 mSv [0.04–21.5 mrem]). Since all calculated TEDE values fall well below the NRC release criteria, the effects of patient or procedure specific factors were deemed insignificant. The NRC regulations for patient release are derived from a patient-specific dose calculation based on the total-body residence time and measured dose rate. Using this method it was determined that all patients treated with Y90 hepatic radioembolization were deemed releasable without instruction or restrictions." @default.
