- W1971334019 abstract "Abstract The long-term-treatment requirements 85 patients presenting with persistent or recurrent urinary-tract infection have been correlated with the competence of hydrokinetic bacterial clearance as measured by the endogenous washout and with the radiological anatomy of the urinary tract. Some correlation was observed between the response to conventional treatment and spontaneous bacterial clearance, those with poor washouts responding least well. High-dose or suppressive therapy was required three times as commonly in patients with poor washouts (groups III or IV) compared with those with good washouts (groups I and II). The differences were, however, not great and the washout test proved to be of little predictive value in the individual subject. The endogenous washout test was of little value in predicting the frequency of reinfection. A markedly higher reinfection-rate was observed in patients with diseased kidneys or abnormal bladder emptying on X-ray than in those with normal kidneys or distensible upper urinary tracts. While hydrokinetic clearance mechanisms have some effect on the ease or otherwise of treatment, other important factors are also involved. Such factors appear to be of even greater importance in determining the frequency of reinfection." @default.
