- W1971637328 abstract "In this issue of Acta Paediatrica Almroth et al. report on a study of exclusive breastfeeding in Lesotho. The study was carried out to determine the need for education in raising the rates of exclusive breastfeeding. Qualitative information was obtained through focus groups and in-depth individual interviews with mothers grandmothers village health workers and paid caregivers. As found in many other investigations Almroth et al. observed that a substantial majority give extra water during lactation making exclusive breastfeeding uncommon. A study 3 y later showed no appreciable change. On the basis of the statements of grandmothers and clinic nurses it is the health providers who are responsible for spreading the practice of water supplementation. The educational efforts recommended and/ or put in place are not dealt with in the communication. The observations made and the conclusions reached raise several questions regarding the size and nature of the problem of the unnecessary supply of extra water to breastfed infants. There are numerous reports from many areas that exclusive breastfeeding is rare but few about what action has been taken and how the populations studied are informed of the results.
