- W1971982873 abstract "HISTORY: 22-year old female Olympic long-track speed skater presented with a 1.5-year history of amenorrhea which began after a piriformis injury. She states that she had a 4.5 kg weight loss following this injury. She had no prior history of menstrual dysfunction and began her period at age 13 y. She was initially treated with anti-inflammatory medication without relief. Following this, her activities were changed to rowing, cycling and running with no strength and sport-specific training, and her team physician modified her skates, which resolved the problem. An initial panel of blood tests was ordered by team physician (see table below). She had also been working with a sports dietitian since 7/ 2005. She denied pregnancy, use of contraception, eating disorder, galactorrhea, heat or cold intolerance, or hirsuitism. Her past medical history was significant for exercise-induced asthma and vocal cord dysfunction. PHYSICAL EXAMINATION: Her vital signs are normal. Her heart, lung, thyroid exam are all normal and she has no physical exam features consistent with eating disorder. DIFFERENTIAL DIAGNOSIS: Amenorrhea due to decreased energy availability Female athlete triad TESTS AND RESULTS: FINAL WORKING DIAGNOSIS: Amenorrhea due to decreased energy availability TREATMENT & OUTCOME: Energy intake (EI) was increased slightly to better meet high exercise energy expenditure (EEE) of intense training. Change to sport-specific training decreased the athlete's EEE, while EI increased, resulting in EA of 41.6 kcal/kgFFM/d and weight, fat, and FFM gain. Resumption of normal menstrual function occurred 3 weeks after progesterone challenge (10 mg of progesterone x 10 d) and continuous intervention adjusting EI to high EEE. This athlete probably resumed normal menstrual function due to improved EA, resulting from slight adjustments in training and nutrition, and less likely due to hormonal manipulations." @default.
