The radiological study of the upper tract is a useful tool in the diagnosis of UUTT. Ureterorenoscopy (UR) detects more accurately the presence of a tumour and it is the established method of followup. Flexible instruments facilitate the surveillance of the upper tract. We evaluate the results of a less invasive followup schedule after the treatment of low grade UUTT treated conservatively. METHODS This is a retrospective study of 36 patients who underwent a UR or percutaneous approach because of low grade (G 1-2) UUTT, 18 as first tumour of the upper tract (Group 1) and 18 after treatment of another ipsilateral or contralateral upper tract tumour (Group 2). 86 % had previous history of bladder cancer. Mean age of 64 years. The strategy of followup was urine cytology, cystoscopy and intravenous (IVP) at 3, 6 and 12 months, then every 6 months for the first 5 years. RESULTS There was a 33.3 % recurrence in both groups. The mean followup of each group was 76 and 94 months. In group 1 the mean time to recurrence was 23.5 months and in group 2, 41 months. There was one high grade recurrence in each group and two developped invasive tumours in group 2. Finally, 6 of the 12 recurences were treated with radical surgery, with a global preservation kidney rate of 83.3 %. Of the patients who recurred (n=12), only one died of the disease (2.7 %). CONCLUSIONS Endourological management of low grade tumours of the upper tract is feasible and with good oncological and functional results. There is no need to perform followup UR unless there is a positive cytology or suspicious or clear tumoral image on IVP.
