Betroffene Patienten haben erhöhte TSH-Spiegel, niedrig-normale oder erniedrigte Trijodthyronin (T3) und Thyroxin (T4) Spiegel und meist eine normale Schilddrüsenmorphologie. In Abhängigkeit vom Schweregrad der Resistenz variiert nicht nur das klinische Erscheinungsbild, sondern auch der Grad der Erhöhung der TSH-Serumspiegel und der Erniedrigung der Schilddrüsenhormon-Serumspiegel. Differentialdiagnostisch sind Patienten mit einer primären Hypothyreose (erhöhtes TSH, erniedrigtes T4 und hypoplastische Schilddrüse) und Patienten mit beginnender zentraler Hypothyreose (niedrig-normales oder erniedrigtes T4 bei noch nachweisbarem TSH) abzugrenzen. Bei fünfzehn Familien mit homozygot rezessiver oder kombiniert-heterozygoter Vererbung wurden Mutationen im TSH-Rezeptor-Gen als Ursache der RTSH nachgewiesen. Die Mutationen im TSH-Rezeptor-Gen führen aufgrund einer veränderten Ligandenbindung bzw. eines Synthese- oder Prozessierungsdefektes zu einer verminderten oder fehlenden Rezeptorfunktion. Der Phänotyp der Resistenz gegen TSH findet sich auch in einer großen Anzahl von Familien, bei denen sich keine Mutationen im TSH-Rezeptor-Gen nachweisen ließen. In den meisten dieser Familien wird der Phänotyp autosomal dominant vererbt, die zugrundeliegende genetische Ursache ist jedoch bislang nicht bekannt. Summary: Resistance to thyrotropin (TSH) is a syndrome of reduced sensitivity to a biologically active TSH molecule. Subjects have elevated TSH levels but no goiter. However, thyroid hormone concentration may vary from normal to very low, depending on the severity of the resistance. Individuals with very high TSH, low thyroxine (T4), and hypoplastic thyroid glands can be mistakenly diagnosed with primary hypothyroidism due to agenesis of the thyroid gland. Those with normal or slightly decreased T4 can be misdiagnosed as having central hypothyroidism, especially if their serum TSH concentration is only slightly elevated. Mutations in the TSH receptor (TSHr) gene have been reported in fifteen families with homozygous recessive of compound heterozygous inheritance. The mutant TSHrs show reduced or no function due to either altered ligand binding or a defect in membrane targeting. A larger proportion of families express the phenotype in the absence of a TSHr defect. In many, the inheritance is dominant and the genetic cause has not yet been determined." @default.
