El paciente fue intervenido bajocirculación extracorpórea, observándose una heridapunzante en la pared libre del ventrículo izquierdo.Se realizó ecografía transesofágica intraoperatoriacomo ayuda para definir la trayectoriaintracardíaca del cuerpo extraño, y para descartarlesiones asociadas por trayactoria múltiple. Se repararonlas lesiones y el paciente fue dado de altasin complicaciones. Este caso ilustra la posibilidadde supervivencia tras un traumatismo penetrantedel corazón, y la utilidad de la ecocardiografía a lahora de diagnosticar y guiar la reparación quirúrgicade estas lesiones. We present the case of a patient who suffered acardiac penetrating trauma due to a 6-cm long steelsplinter. He was self-admitted to the emergencyroom and was asymptomatic. Cardiac trauma wasdiagnosed by the presence of a foreign body in hischest X-ray. Transthoracic and transesophagealechocardiography showed pericardial effusion anda dense foreign body that crossed the left ventriclefrom upside down and forward to back. The patientunderwent cardiac surgery under extracorporal circulation.A shooting wound was seen on the leftventricular free wall. Transesophageal echocardiographywas performed during surgery in order todefine the position of the foreign body and to discardlesions due to multidirectional injury. Lesionswere repaired and the patient was discharged withno complications. This case report illustrates thepossibility of survival after cardiac penetratingtrauma, and the role of echocardiography in thediagnosis and surgical repair of this type of trauma." @default.
