Primero se realizó una descripción del perfil psicopatológico, siguiendo los criterios del DSM-IV, de los 20 casos de mujeres con historia de incesto, detectados durante el período 1998-1999 en un sector de población de 19.431 habitantes y, posteriormente, se realizó un ensayo de tratamiento con 6 pacientes que cumplían los criterios de participación establecidos. Se tomaron medidas repetidas antes y después de la intervención, utilizando: a) entrevistas semiestructuradas y declaraciones espontáneas, y b) puntuaciones en las escalas de depresión (BRDS), panic attacks (PASQ), ansiedad estado y rasgo (STAI) y adaptación social (SASS abreviada). Se utilizó una intervención terapéutica multifocal que combinaba la terapia farmacológica con la individual, la familiar y la de grupo. Cuatro pacientes de las 6 iniciales completaron el tratamiento y presentaron una mejoría altamente significativa en todas las medidas clínicas (p = 0,0328). Las mujeres que sufrieron abuso sexual intrafamiliar en la infancia pueden presentar serios problemas de salud mental en la edad adulta, que van desde el transtorno de angustia a la depresión crónica. Tanto el tipo de patología como los sucesos desencadenantes en el entorno parecen tener relación no sólo con el contexto y las variables relacionadas con el incesto, sino también con los requerimientos biopsicosociales correspondientes a la etapa evolutiva en que aparecen. Por ello, la respuesta a la terapia puede ser más eficaz si abordamos los distintos aspectos evolutivos y biopsicosociales del problema utilizando un programa de tratamiento multifocal. To evaluate the clinical profile and therapeutic possibilities in adult women consulting for mental health problems who had been victims of intrafamily sexual abuse during childhood. From a population of 19,431 inhabitants, we evaluated a sample of 20 women with a history of incest detected during a 2-year period (1998-1999). A multifocal therapy program (MTP) was performed in 6 patients who met the criteria established for participation. Repeat measures before and after the MTP were taken using the following instruments: a) semi-structured interviews and self-reports, and b) the Bech-Rafaelsen Depression Scale (BRDS), the Panic Attack Symptoms Questionnaire, the State-Trait Anxiety Inventory (STAI) and the Social Adaptation Self-evaluation Scale (SASS). The MTP combined drug therapy with individual, group and family therapy. Of the 6 initial participants, 4 completed the program and all four showed significant improvement in the clinical parameters measured (p = 0.0328). Women with a history of incest in childhood may have serious mental health problems later in life, ranging from panic disorder to chronic depression. We observed a relationship between clinical symptoms, the stage of life in which these appear or increase its intensity, and the life events leading these women to ask for help. Therefore, treatment response may be more effective if the biological, psychological and social factors involved in the problem are approached using a multifocal approach." @default.
