- W1974476105 abstract "Improvements in the management of testicular germ-cell tumours (TGCT) over the last 25 years have been one of the major successes of modern oncology. Underpinning its success has been the combination of active chemotherapy with staging and prognostic information provided by serum tumour markers and cross-sectional imaging. Over recent years attention has focussed on not only improving cure for those patients where treatment is currently unsuccessful but also on the potential cost of this form of treatment (e.g. second malignancy, cardiovascular disease) in this young population. As we head into the 21st century one avenue that is being actively explored is whether functional imaging, and in particular [F]-labelled fluoro-deoxy glucose positron emission tomography (FDG-PET), could have significant advantages over anatomically derived imaging. Computer tomography (CT) has been the imaging modality of choice in TGCT with a role in initial staging of patients, in assessment of disease response and detection of relapse. However, reliance on structural abnormalities to identify disease, means CT suffers from a number of limitations. It cannot, for instance, detect disease in ‘normal-sized’ lymph nodes or detect whether there is active tumour in residual masses after chemotherapy. These difficulties limit the accuracy of CT in assessing of TGCT. A prime example of this is the knowledge that approximately 25% of patients will have normal-sized para-aortic nodes containing tumour, and some patients with enlarged nodes will not [1,2]." @default.
