- W1975331898 abstract "HAVING worked in a general medical unit we were aware of the problems of communicating to the multidisciplinary team the stages of rehabilitation achieved by patients who had suffered a cerebrovascular accident (CVA). In this type of unit, medical and nursing staff are coping with a variety of medical problems of varying degrees of severity and perhaps, in their eyes, of importance. We felt that in this type of setting the area of stroke was greatly neglected and misunderstood. Although meetings provided the basis of communication we felt there was a need for a visual record which would afford immediate and easily observable information about each stroke patient. It was our belief that the provision of such a system would not only foster improved communication but would also enable us to implement an educational programme in the unit for madical and nursing staff as well as relatives. We carried out a computer and literature search into the use of visual symbols as a means of communication in the area of hemiplegia. Although there are recognised assessment forms, we found no such communication system to be in operation. A set of 12 symbols based on the human figure was designed. These denote the following: neglect, painful shoulder, static and active sitting and standing balance, different stages of dressing, three recognised lifting techniques and a symbol for 'no walking'. The Charting System comprises of stickers (24 on each sheet), booklets which are administered to all members of staff and relatives, posters which are placed throughout the ward, and a video-tape. In Gartnavel General Hospital each CVA patient is assessed the day after admission. The name, date of onset and side of stroke are noted at the head of a luminous pink card. Following assessment, what is observable is noted by affixing the appropriate sticker on the card horizontally. Continual assessment is carried out and any change is noted by the appropriate symbol and dated. The chart is sited above the patient's bed where it can be easily observed (see picture). The system is now in use in hospitals throughout the United Kingdom, and further information is available from the authors.
