Maintaining bowel function in geriatric patients who are hospitalized for long periods of time is a major duty and responsibility of the nursing staff. Yet, few nurses have any systematic approach to help patients to establish regular bowel habits with minimal medication. Why are geriatric patients in long-term care facilities such excellent candidates for constipation? Often constipation can be related to decreased physical activity, inadequate bulk and fluid in the diet, stress, or constipating medications (Sklar, 1972). According to Sklar, the gastrointestinal problems of older patients do not differ greatly from those of the young and constipation can not be attributed to the ‘wearing out’ of the digestive tract. Although there is a tendency for gastric secretion to decrease with age, there is no evidence that hypochlorhydria induces symptoms. The output of other digestive enzymes may decrease, but ample quantities remain for digestive function (Spence, 1961). References as late as 1976 (Clark and Scott, 1976) refer to ‘dyschezia’ in the aged. Yet, this condition of atonic, lax colon with decreased peristalsis has been difficult to document in clinical research and despite considerable progress in electrophysiology and pharmacology of the colon, regulation of human colonic motility in health and disease is still not well understood (Misiewicz, 1975).
