À long terme, le succès du traitement par la greffe de cellules souches hématopoïétiques est surtout affecté par la récurrence de la maladie, le rejet de greffe ou une maladie du greffon contre l’hôte. Dans ce cadre, les analyses de chimérisme permettent d’évaluer la prise de greffe mais également de découvrir très précocement la rechute. Des études rétrospectives ont démontré l’intérêt de cette analyse sur le devenir des patients greffés. De plus, l’analyse du chimérisme sur des populations cellulaires spécifiques triées a également établi que l’on pouvait augmenter la sensibilité de détection de la rechute de la maladie. La détermination du chimérisme permet d’intervenir efficacement avant l’apparition des signes cliniques chez les patients atteints d’hémopathies malignes ayant un haut risque de rechute et également de contrôler l’expression de la maladie résiduelle lorsque les marqueurs moléculaires spécifiques sont absents. Haematopoietic stem cells transplantation, widely used these last decades, represent the ultimate treatment resource for patients with haematological malignancies. Long range success of this treatment is particularly affected by relapse of the initial disease, graft rejection or graft versus host disease. Chimerism analysis after transplantation had been used since several years to document engraftment, to determine the risk of relapse and to adapt therapy promptly when necessary. Usefulness of this analysis for the outcome of transplanted patients, as well as the impact of using high sensitive techniques coupled with specific cell populations sorted have been demonstrated by retrospective studies. Follow-up of chimerism would allow to operate efficiently before the onset of clinical signs in leukaemic patients with high risk of relapse and to control the expression of minimal residual disease when specific molecular markers could not be monitored." @default.
