• W1976677072 abstract "Purpose: To evaluate the combination of EUS and secretin stimulation in patients with suspected chronic pancreatitis at our institution. Methods: With each patient, a test dose of synthetic secretin was given to assess for the presence of adverse reactions. Subsequently, a dose of 1 U/kg was administered just prior to the start of the EUS. Duodenal aspiration was performed at 15 minutes and again at 30 minutes. EUS criteria (n/11 per Catalano, et al) for chronic pancreatitis were recorded for each patient. Results: Data has been collected from 26 patients. The mean age of the patients was 53.5. Eight patients were male and eighteen were female. The mean bicarbonate concentration at 15 minutes was 56.69 (95% C.I. 49.65, 63.73). The mean bicarbonate concentration at 30 minutes was 46.65 (95% C.I. 38.79, 54.51). Comparing the 15-minute data set to the 30-minute data set, using the paired t-test and setting the null hypothesis to no difference (Ha: mean (diff) = 0), the null hypothesis was rejected (P = 0.0178). The mean number of EUS findings was 3.35 (95% C.I. 2.88, 3.82). There were no adverse reactions seen in this patient population. Conclusions: Secretin stimulation can be added to EUS without significant added time or risk. We compared the mean bicarbonate concentration at 15 minutes to that at 30 minutes. We were interested in assessing whether or not measurement at 15 minutes alone might be adequate for diagnosing pancreatic insufficiency. In our analysis, the two means were both well below established thresholds for pancreatic insufficiency, but we could not conclude that the two means were similar. There was greater disparity between 15-minute values and 30-minute values at higher bicarbonate concentrations. We also compared the results of the secretin stimulation test to the number of EUS findings, but we were unable to conclude at this point whether or not there is a correlation between the two.
