- W1977158936 abstract "Introduction. Since the beginning of cardiac surgery, the EEG has been used to monitor brain electrical activity and detect signs of cerebral ischemia. Despite numerous investigations and improvements in EEG equipment and analysis techniques, its usefulness during cardiac surgery remains very controversial. [1] In this evaluation of 216 patients, we examined the relationship between intraoperative QEEG changes and neurocognitive decline 6 weeks following surgery. Methods. Following IRB approval and informed consent, elective CABG patients (n=216) underwent standardized CPB (nonpulsatile, alpha-stat pH, membrane oxygenator, arterial-line filter, temp 32[degree sign]C (n=125) or 35[degree sign]C (n=91) using continuous (steady-state) midazolam/fentanyl infusions. Conventional 19-channel QEEG were obtained using the Spectrum 32 (Cadwell Labs) and CIMON QEEG analysis, with Critical QEEG Changes [2] being defined as: 3-standard deviation increase in relative low frequency (1.5-3.5 Hz) power persisting for >or=to 5 min. A battery of 11 cognitive tests were performed preop and 6 weeks postop. To determine cognitive decline, two outcome measures were used: 1) Cognitive Deficit = >20% decrease on >20% of cognitive tests, 2) Cognitive Impairment Index (CII), average % decline from baseline scores on all tests. Association of Critical QEEG Changes with CII was tested with Spearman correlation and linear regression to account for age. The categorical outcome of Cognitive Deficit was tested with logistic regression adjusted for age. Results. The incidence of Cognitive Deficit at 6 weeks was 35.2% and CII showed an average 11% decline. Critical QEEG Changes occurred in 32% of patients. No significant association was found between Critical QEEG Changes and neurocognitive decline at 6 weeks. Discussion. Many patients (32%) met our criteria for Critical QEEG Changes; however, this could not be related to postoperative neurocognitive deficits. This may be explained in part by the lack of specificity of the CIMON method of QEEG analysis which may not distinguish between the effects of cerebral ischemia and the various confounders.
