- W1977360119 abstract "PET with 18F-misonidazole (F-MISO) is a non-invasive method of depicting tumor hypoxia. A prospective clinical trial of F-MISO PET/CT for patients scheduled for radiation therapy (RT) was conducted, in order to detect intratumoral hypoxic areas and to visualize reoxygenation during fractionated RT. To date, eight patients including three with head and neck cancers, three with gastrointestinal cancers, one with non-small cell lung cancer, and one with uterine body cancer were included. The maximum tumor size ranged from 14 mm to 68 mm. All patients underwent FDG PET/CT prior to RT. As a protocol, F-MISO PET/CT was performed twice before RT with approximately 20Gy/10 fractions. PET scan was obtained 3h after injection of 3.7MBq of F-MISO. We measured F-MISO maximum standardized uptake values (SUVmax) of normal muscles (bilateral neck, back, and buttock muscles) in these eight patients. For each study, F-MISO SUVmax was measured in both primary tumors and metastatic lymph nodes. In addition, the tumor-to-muscle ratios (T/M) of F-MISO SUVmax were measured. Six of the eight patients underwent FMISO PET/CT twice, before and during fractionated RT. In total, 14 F-MISO PET/CT studies were performed. Mean ± SD of SUVmax of the normal muscles was 1.26 ± 0.17. Based on these data, accumulation of F-MISO above a value of mean + 2SD (1.60 SUV) was regarded as indicating a hypoxic area. For six of the eight patients, F-MISO SUVmax prior to RT was above 1.60. Except in one patient with postoperative recurrent uterine body cancer, the remaining five tumors showed a decrease in SUVmax or T/M ratio after approximately 20Gy of fractionated RT. In terms of initial tumor response, two tumors with SUVmax <1.60 prior to RT showed CR. Two other tumors with a significant decrease in SUVmax or T/M ratio in the 2nd F-MISO study also showed CR. Three tumors with high SUVmax >2.5 in the 2nd study showed PR or NC. For the remaining one patient, RT was terminated at 42Gy due to acute toxicities, and tumor response could not be evaluated. Based on the SUVmax of the normal muscles, accumulation of F-MISO above 1.6 SUV was regarded as indicating a hypoxic area. Decrease in the SUVmax or T/M ratio, suggesting reoxygenation, was observed for most tumors after approximately 20Gy of fractionated RT. SUVmax of F-MISO before or during fractionated RT may predict the clinical outcomes of the tumors." @default.
