- W197797046 abstract "Depression and bipolar disorder are two of the commonest CNS disorders in the developed world. While some individuals can be treated effectively with available drugs, many do not respond, especially in the depression associated with bipolar affective disorder (BPAD). Drugs acting on monoaminergic neurotransmitter systems have been the cornerstone of treatment for several decades and their mechanism of action remains poorly understood. However, despite the phenomenal success of these drugs for treating affective disorders they have many shortcomings and problems that are associated with all drugs used to treat affective disorders. Thus, the treatment of depression and bipolar depression still represents a relatively understudied area in clinical psychiatry. The depressive phases of BPAD can be very disabling, with significant associated comorbidity and suicide risk, impairment in function, and impaired quality of life. Although the treatment of depression and bipolar depression can be a complicated clinical task, the treatment armamentarium is rapidly expanding. The selective 5HT reuptake inhibitors (SSRIs) and lithium are currently the gold standards and are the most appropriate initial treatment options for depression and the depressive phase of bipolar disorder, respectively. Other mood stabilizers with demonstrated preliminary efficacy are also discussed in this chapter. Of the current antidepressants, bupropion and the SSRIs may be associated with less risk of inducing hypomania, mania, and rapid cycling compared with tricyclic antidepressants (TCAs), whereas monoamine oxidase inhibitors (MAOIs) are gaining use in patients with anergic bipolar depression. Current paradigms for the treatment of affective disorders, unmet clinical needs, and new research areas are reviewed in this chapter." @default.
