- W1978334876 abstract "Les intoxications aigües par les pesticides organophosphorés (OP) sont responsables d’une lourde mortalité mondiale, en particulier dans les pays en voie de développement à fort potentiel agricole. L’essentiel de cette mortalité concerne les ingestions dans un but suicidaire. Ces produits OP sont responsables d’intoxications de gravité variable selon la nature du composé et des quantités ingérées. Le tableau clinique résulte d’une inhibition des cholinestérases aussi bien de la jonction synaptique des fibres du système nerveux central que des érythrocytes ou du plasma (appelées alors pseudocholinestérases). Il comporte classiquement trois syndromes : muscarinique, nicotinique et encéphalique pouvant coexister chez un même malade. La prise en charge précoce et adéquate nécessite des moyens de réanimation symptomatique pas toujours disponibles dans les pays aux ressources limités. Le traitement antidotique comporte l’administration d’atropine et d’oximes. L’administration systématique de ces derniers reste cependant un sujet de controverse, vu leur efficacité inconstante, dépendante de certains paramètres tels que le type d’OP ou le délai d’administration. La décontamination digestive par lavage gastrique pourrait être utile uniquement dans l’heure suivant l’ingestion et après stabilisation du patient. Le charbon activé n’a pas de place au vu des dernières études. D’autres thérapeutiques prometteuses sont en cours d’évaluation comme l’alcalinisation ou l’administration de sulfate de magnésium. Organophosphorus poisoning (OP) represents a major clinical concern responsible of persistent elevated mortality in the developing countries and resulting in frequent emergent hospital admission. Acute OP toxicity is primarily caused by the inhibition of cholinesterases. Diagnosis is based on specific clinical features including muscarinic and nicotinic overstimulation as well as reduced activity of both erythrocyte acetylcholinesterases and plasma butyrylcholinesterases. Treatment should firstly guarantee adequate breathing and circulation. Control of excessive neurostimulation is obtained with the administration of atropine sulphate as muscarinic receptor antagonist and an oxime as a cholinesterase reactivator, in spite of the lack of evidence-based data. Gastrointestinal decontamination should only be considered for patients who present within 1 h after the ingestion of a significant OP amount and only after stabilisation. Regarding activated charcoal, there is no evidence for its efficiency. The interest of several affordable therapies including magnesium sulphate and alkalinisation that may complete present therapies is currently investigated." @default.
