- W1978396954 abstract "The Metabolic Syndrome and Primary Careby Christopher D Byrne Paperback ISBN 978 0 470 06102 2 Hardback ISBN 978 0 470 51217 3 www.wiley.com Alan Begg General Practitioner and Honorary Senior Lecturer*, * Townhead, Links Health Centre, Montrose, UK Described as a handbook for the primary care team, it has been developed from a much larger text co-edited and part written by these respected authors. Although condensed, much has been packed into the 135 pages with the opportunity taken to update the text as well as incorporating more recent trial evidence. Metabolic syndrome is defined as a cluster of risk factors and, although obesity was not one of Reaven's original list of risk factors, abdominal obesity is stated as the first feature to appear. Primary care needs to identify individuals with abdominal obesity and both measure and manage their other risk factors. The authors have avoided using or describing the term cardiometabolic disease. The book is divided into four chapters and, although each varies significantly in length, the clearly defined headings guide the reader through each chapter in a logical manner. A chapter on the diagnosis follows an introductory overview chapter. The prevalence and factors affecting these and the revised diagnostic criteria are set out in detail. Chapter three looks at the mechanism underlying the pathogenesis and the consequences in the context of the four associated clinical conditions— cardiovascular disease, type 2 diabetes, polycystic ovarian syndrome and non-alcoholic fatty liver disease. The final and longest chapter covers patient management. Although risk factor management is important it does not provide any magic answers on how to bring about lifestyle change. A comprehensive overview of the interventional studies for the prevention of type 2 diabetes is interesting, as is a section on drugs that influence features of the metabolic syndrome. When discussing drug therapy for polycystic ovarian syndrome the authors sensibly point out that metformin and the glitazones are presently not licensed for this indication. For primary care team members this book provides all they need to know on this topic. It is well illustrated with good, clear yet simple diagrams and it is laid out in an easy to follow manner. Without doubt its main strength is the extensive and full referencing which is an ideal starting point for those intent on more in depth study on this subject." @default.
