- W1978598257 abstract "Mild Cognitive Impairment (MCI) is considered a transitional stage between healthy aging and dementia. Both amnestic and non-amnestic subgroups of MCI have recently been proposed, but few studies have investigated non-amnestic MCI. To compare gray matter atrophy patterns and neuropsychological differences in two single-domain MCI subgroups: dysexecutive and amnestic MCI. Dysexecutive MCI (dMCI) was operationally defined as patients who reported recent onset of dysexecutive symptoms (e.g., attention, multi-tasking, behavior) or had impaired scores on executive, but not memory, measures. Amnestic MCI (aMCI) patients had impaired scores on memory, but not executive function. An extensive battery of neuropsychological tests was administered to characterize the subjects. We used 1.5 Tesla T1-weighted MR images and investigated differences in the gray matter atrophy of 33 dMCI patients, 24 aMCI, and 33 healthy controls using voxel-based morphometry (VBM) in SPM5. We included age, gender, and total intracranial volume as nuisance variables. Data were preprocessed using the SPM5 unified segmentation process and analyzed on a voxel-wise basis at p<0.001, uncorrected. Based on a priori hypotheses, we restricted our analysis to regions of interest in the prefrontal cortex, medial temporal lobe, posterior cingulate gyrus, and precuneus. We found that dMCI patients had more atrophy in the left prefrontal cortex when compared to controls. In contrast, aMCI patients had more atrophy in the bilateral medial temporal lobes, right posterior cingulate gyrus, and right precuneus than controls. Compared to aMCI patients, dMCI patients showed more atrophy in the left caudate nucleus, at a p<0.0001, uncorrected threshold. Conversely, aMCI patients showed more atrophy in the right precuneus than dMCI patients. Compared to controls, dMCI patients scored significantly lower on measures of executive function (i.e., Trailmaking, Stroop, FAS, Digit Symbol), whereas aMCI patients scored significantly lower on measures of memory (i.e., delayed recall of Visual Reproductions) (all p<0.05). We conclude that two distinct, single-domain subgroups of dMCI and aMCI exist, and these subgroups present with spatially different patterns of gray matter atrophy compared to controls. Patterns of impairment on neuropsychological assessments also support the existence of two distinct MCI subgroups." @default.
