- W1979445908 abstract "Conflict of interest: none declared. About 5% of patients with psoriasis suffer from a rare subtype called palmoplantar psoriasis (PPP), limited to the palms and soles. This disabling condition is characterized by yellowish itching pustules, desquamation, painful recurrently bleeding fissures, and local inflammation. Many treatments have been used but none is accepted generally as being reliably effective.1 Unfortunately, there seems to exist a widespread opinion among physicians that there is ‘no cure’ for PPP. We describe the case of a 35‐year‐old man, presenting symptoms (Fig. 1a,b) consistent with severe PPP restricted exclusively to the palms and soles and impairing mobility. No other dermatological conditions were found at the time of diagnosis, although the patient stated that he had experienced occasional endogenous eczema in adolescence. Additionally, there were signs of onycholysis of the fingernails, also suggesting the psoriatic origin of the lesions.2 There was no family history of psoriasis, and the patient had a history of stress, frequent episodes of dyspepsia and generalized arthritic pain compatible with psoriatic arthritis (patient was negative for rheumatoid factor). Alcohol and tobacco consumption habits were moderate. The patient, interestingly, reported a 6‐year history of misdiagnosis of the lesions and respective mistreatment including contact eczema and fungal infections. Several months of separate treatments with topical corticosteroids (betamethasone dipropionate 0.05% as cream under hydrocolloid occlusion over night for 4 weeks) and vitamin D3 derivates (calcipotriol 50 μg/g once daily for 1 month) were effective only in temporarily reducing symptoms, and marked relapses had occurred upon withdrawal, particularly with corticosteroids." @default.
