- W1979532085 abstract "Objectives: Embryonic production and secretion of FR has a positive role in the implantation process (Fertil Steril, 1998, Suppl, S58). We evaluated the FR secretion and potential for implantation of oocytes fertilized by different meiotic stages of the male gamete. Design: We compared the fertilization rate, FR secretion and implantation potential of oocytes injected with ejaculated spermatozoa showing motility >10% (9 couples; group A), testicular spermatozoa demonstrating motility >20% recovered from obstructed azoospermic men (4 couples; group B), testicular spermatozoa showing motility >10% recovered from non-obstructed azoospermic men (NOAM; 14 couples; group C), testicular spermatozoa exposing motility 0–1% recovered from NOAM (14 couples; group D), round spermatids (7 couples; group E), secondary spermatocytes (7 couples; group F), or primary spermatocytes (12 couples; group G). Materials and Methods: Mature oocytes were collected from healthy young women (considered fertile after careful evaluation of the genital tract) and injected with different stages of the male gamete (groups A, B, C, D, E, F, and G). In groups F and G, mechanical and chemical ooplasmic stimulation were applied during and after the ooplasmic injection, respectively. Injected oocytes were cultured for 18 hours. At the end of the culture period, the culture medium was changed and oocytes were cultured separately for additional 54 hours. At the end of the new culture period the soluble FR concentration in the culture medium was evaluated using ELISA. Up to three fertilized oocytes were transferred to each respective female partner. Results: Fertilization rate was not significantly different (P>0.05) among groups A (64%), B (65%), C (58%), and D (56%). However, it was significantly smaller in groups E (30%), F (21%), and G (8%) (P<0.05). Pregnancy rate and mean FR concentration/oocyte were significantly smaller (P<0.05) in groups D (22%, 994 ng/ml), E (0%, 457 ng/ml), F (0%, 303 ng/ml), and G (0%, <10 ng/ml) than in groups A (44%, 1760 ng/ml), B (50%, 2 160 ng/ml), and C (42%, 1510 ng/ml). Analyzing the cycles of groups A, B, C, D, E, F, and G in which each one of the transferred oocytes secreted FR less than 500 ng/ml, it was found that a level of FR secretion/oocyte <500 ng/ml could predict failure of implantation with 100% accuracy. Conclusions: The low FR secretion from embryos generated from the fertilization of oocytes with immotile sperms, round spermatids, secondary spermatocytes, or primary spermatocytes may contribute to their low potential for implantation. The stage of differentiation and viability are the parameters of the male gamete that affect embryonic FR secretion and subsequently capacity for implantation. Considering that embryos secreting FR less than 500 ng/ml did not implant, it appears that there may be a threshold level in embryonic FR secretion below which a further decrease reduces embryonic capacity for implantation. Furthermore, measurements of embryonic FR secretion may have a role in choosing embryos for transfer when more than 2 or 3 oocytes are fertilized.
