- W1979644024 abstract "Increasingly, postoperative cognitive dysfunction (POCD) is recognized as a complication after surgery in the elderly. To date, the etiology of POCD remains unclear, although advanced age and a cognitive impairment preoperatively are factors associated with this disorder. We retrospectively examined cognitive functioning in individuals with mild cognitive impairment (MCI) and normal controls with a history of surgery under general anesthesia and those without a surgical history. We hypothesized that elderly patients with MCI would have an accelerated progression of dementia postoperatively when compared to matched controls. We examined cognitive functioning in 169 longitudinally studied individuals (48 subjects with MCI and 121 normal controls) from the NYU Alzheimer's Disease Center and Center for Brain Health. Individuals who received at least three clinical evaluations were selected for this study. Neuropsychological testing included the Guild Memory Test, subtests from the Wechsler Memory Test-R, the NYU List Learning Test and the Object Naming Test. Surgery occurred prior to the first or second clinical evaluation. Cognitive performance was measured at each time point and the change over time was examined. Measures were z scored based on the NYU Robust Cognitive Norms. Cross sectional and longitudinal change were examined for the following cognitive domains: working memory, immediate verbal memory, delayed verbal memory, and attention domains. No demographic differences were found within each diagnostic group for individuals with and without surgery. Normal controls with (n = 50) and without (n = 71) surgery did not show longitudinal performance differences in any cognitive domain. Individuals with MCI showed an effect of surgery in the working memory domain which included digits forward and backward. MCI with surgery (n= 14) had a significantly greater decline in performance on the digit span forward test compared to MCI without surgery (n = 34) (F [1,46] = 8.1, p < .05) over the first and second evaluation. Interestingly, digit span forward performance improved at the third time point. Surgery under general anesthesia negatively impacted working memory in MCI but not in normal controls. Performance recovered within approximately four years post surgery suggesting that these performance declines are not permanent." @default.
