- W1980951936 abstract "fter the flap has been placed, the vascular pedicle is draped own into the neck and care is taken to ensure that it is not wisted or kinked. The site where the surgeon intends to make he incision in the recipient vein should be decided by first ligning the donor vein to the recipient vein in a passive osition. It is then common practice to apply clamps across he recipient vessel before making the anastomosis. It is our xperience that it is easy to lose the desired site for the venoomy because of changes in orientation that can ensue after pplication of the clamps. We therefore mark the site of the enotomy with blue ink (Fig. 1). We then insert an 8/0 suture t this point, tie it, and leave the ends long. The clamps are hen applied in the usual way and the site for the venotomy emains easily visible. To create the venotomy, rather than following the tradiional method of picking up the vessel wall with jewellers orceps, we pick up the ends of the suture (Fig. 2) to crete a tenting effect to allow the venotomy to be created with harp microdissecting scissors in a controlled and atraumatic ashion.1–4" @default.
