- W1981001015 abstract "Abstract The selection of women for an exenteration procedure depends initially on the pattern of referral to a centre of excellence. The majority of these operations are carried out on women with central recurrent disease in the pelvis following primary treatment with radiotherapy. A smaller number of cases are operated on for carcinoma of the vulva or vagina. Exenteration is intended as a curative operation although occasionally it is done as a palliative procedure when a fistula is present. The assessment and counselling of patients is vital to the success of the procedure. The final decision to proceed will often only be made after a detailed intra-operative review of the upper abdomen and frozen section of all of the pelvic lymph nodes. Only if the findings are negative is it justified to proceed. The reconstructive procedures include the formation of an ileal conduit and colostomy and in some cases the formation of a neo-vagina. It is recommended that there is a team approach for assessment, operation and postoperative care by a gynaecologist, a urologist and a colorectal surgeon. Strict observation of these criteria should produce a cure rate approaching or even exceeding 50%." @default.
