- W1981562689 abstract "Controversy exists as to whether regional blocks should be performed before of after induction of general anaesthesia in adults [1–3]. We read with interest the article by Feely et al. [4], as we recently completed a similar survey. All grades of anaesthetists working at Heartlands and Solihull Hospitals during June and July 2008 were surveyed. We gathered data on spinal anaesthesia, thoracic and lumbar epidurals, interscalene, lumbar plexus, sciatic, femoral and ankle/foot nerve blocks. We asked anaesthetists whether they routinely performed these blocks awake, sedated or under general anaesthesia in adults. They were asked to give reasons for their choices and were free to add comments. Fifty-nine out of 71 anaesthetists responded (83%): 31 consultants, seven non-consultant career grades and 21 ST1-7 trainees. Central neuraxial blocks were performed before induction of general anaesthesia by 85% of anaesthetists (spinals by 86%, thoracic epidurals by 91% and lumbar epidurals by 81%). Interscalene blocks were performed before induction of general anaesthesia by 50% of anaesthetists. Lower limb blocks were performed before induction of general anaesthesia by only 9% of anaesthetists (lumbar plexus blocks by 7%, sciatic nerve blocks by 14%, femoral nerve blocks by 12% and ankle/foot blocks by 5%). There were no differences in practice between the different grades of anaesthetists. The main reason stated for performing any block before induction of general anaesthesia was patient safety, whereas that for performing any block under general anaesthesia was patient comfort. In our department, practice with regards to interscalene block was divided equally amongst the anaesthetists. There was a clear difference of opinion in the perceived risks when performing central neuraxial blocks compared with lower limb blocks under general anaesthesia. Our results for the timing of central neuraxial blocks and lower limb blocks are similar to those of the Oxford region survey [4]." @default.
