- W1981650399 abstract "Introduction: Patients with coeliac disease (CD) must adhere lifelong to a gluten-free diet to alleviate the disease. The presence of specific antibodies in their serum, IgA anti endomysium antibodies (EMA), and IgA anti human tissue transglutaminase antibodies (tTG ab), is significant diagnostic evidence for the disease; it is nearly 100 % specific with a sensitivity of between 96 – 98 % (1,2). However, in this study, we report on the long-term follow up of a small group of symptomatic children with CD who have not adhered to a gluten-free diet, and have developed tolerance to gluten. Methods: Usually, serum antibody titres of EMA and IgG- and IgA-anti gliadin antibodies (AGA) were assessed before, or at the same time as a small bowel biopsy procedure. Twelve patients (12/926) with positive titres of EMA had a normal small bowel mucosa. Five of them (2.1–11.1 years of age mean age: 4.4) were evaluated clinically and serologically at regular intervals. Relatives of two of them had CD. In November 2003 all of the patient’s sera were reassessed; serum tTG ab titres were determined. Results: At presentation, the five children’s serum had high titres of EMA/tTG ab, surprisingly, the number of intra-epithelial lymphocytes was normal, and, the mucosal architecture of their small bowel was normal. Because of the latter, a normal diet was continued, followed by regular clinical and serological check-ups. During the course of the study, 4.1–7.1 years (x: 5.7 years), negative seroconversion of EMA and tTG ab occurred within 4–21 months (x: 10.2 months) and persisted. Clinical symptoms disappeared along with the disappearance of the antibodies. In contrast to children with positive EMA and a flat mucosa, IgA AGA titre was normal in all five patients, while the positive IgG AGA normalized in three patients and lowered their titre in two. Total serum IgA was normal in all patients. Conclusion: For the first time, we report the development of an immunologic and clinical tolerance to gluten in a small group of patients with typical clinical symptoms of coeliac disease, high titres of EMA and tTG ab, and a normal architecture of their small bowel mucosa." @default.
