The interrelations between the fine structural changes of the lungs following perfusion and the early postoperative courses of the patients were studied. Only mild edematous changes were observed in eight patients with the light microscope, but electron microscopic examination disclosed various changes in the fine structure of the lungs after perfusion in most of the cases, such as dispersion of the alveolar septa and the pericapillary spaces, rarefaction and of the cytoplasm of type I and type II pneumonocytes with swelling of the mitochondria and endoplasmic reticula and vacuolation of the capillary endothelial cells. In cases in which the full perfusion time exceeded 150 minutes, these findings were remarkable. The postoperative arterial PO2 in these cases remained low even with intensive respiratory care.
