- W1982136956 abstract "Study Objective: Chronic endometritis that maybe a cause of recurrent abortion in which endometrium is not ideal place for zygote growth. Presence of stromal edema, hyperemia, or endometrial micropolyps (< 1 mm in size), are hysteroscopic presentations of endometritis. This study attempted to detect Correlation of hysteroscopic findings and pathologic results of endometritis in patients with unexplained RSA by doing fluid hysteroscopy and pathologic evaluation of suspicious sites; Design: Cross-sectional study. Setting: University hospitals. Patients: 142 patients with RSA, in whom hysteroscopy was indicated, underwent fluid hysteroscopy followed by endometrial biopsy from suspicious areas. Intervention: Hysteroscopy. Measurements and Main Results: Of 142 women 43% had primary and 57% had secondary RSA. Ninety-six women (67.6%) had positive signs of endometritis found in hysteroscopy; and in 46 patients (32.4%), none of the hysteroscopic signs of endometritis was seen. Out of those 96 women with positive hysteroscopic sign(s), in 61 patients (63.5%) endometritis confirmed pathologically; and the rest (36.5%) had no positive pathology (p < 0.05, positive predictive value = 63.54%, and negative predictive value = 97.82%). This study showed sensitivity of 98.4%, and specificity of 56.25% for hysteroscopic findings of endometritis. Overall the incidence of endometritis (pathologically confirmed) was 42.95% in unexplained RSA. As our study also showed,that out of 62 patients with positive hysteroscopic and pathologic results, 14 women (22.6%) had primary unexplained RSA, but 48 patients (77.4%) were those with secondary RSA (p < 0.05%). Conclusion: Diagnostic hysteroscopy could be highly sensitive (98.4%) for diagnosis of endometritis in women with unexplained RSA.Our study, which was the first study because of large number of patients with RSA underwent diagnostic hysteroscopy for detection of endometritis, also showed significant relation between secondary unexplained RSA and presence of endometritis. Therefore, endometritis should be considered as an important cause of recurrent abortion in unexplained RSA." @default.
