- W1982161862 abstract "咳嗽は持続期間により分類されている．3週間以内の咳嗽を急性咳嗽，8週間以上続く咳嗽を慢性咳嗽という．3週間以上8週間未満の咳嗽は，遷延性咳嗽，subacute cough（亜急性咳嗽）と呼んでいる．本邦における遷延性・慢性乾性咳嗽の4大原因疾患は，咳喘息，アトピー咳嗽，かぜ症候群後咳嗽（感染後咳嗽），胃食道逆流による咳嗽である．遷延性・慢性湿性咳嗽の原因の大部分は，副鼻腔気管支症候群である．かぜ症候群後咳嗽は，別名，感染後咳嗽といい，ウイルスや肺炎マイコプラズマ，肺炎クラミジア感染後に咳嗽が遷延する病態である．ここには百日咳による咳嗽も含まれる．かぜ症候群後咳嗽は，遷延性咳嗽（亜急性咳嗽）の主要な原因疾患で，自然軽快傾向のある疾患である．一方胃食道逆流（GER）は，その名の通り，「胃内容物が食道に逆流する」ことである．胃食道逆流は，生理的現象である．胃食道逆流症（GERD）は，胃食道逆流により，何らかの症状や組織障害を伴う場合に使用される．胃食道逆流による咳嗽は，咳嗽を主な症状とし，胸やけなどの胃食道逆流症状の訴えがはっきりしない場合もある．診断には，24時間食道pHモニターが感度，特異度ともに優れている．しかし侵襲的検査であり，一般臨床上普及していない．他の遷延性・慢性咳嗽の原因が否定され，Empirical therapyとしてのプロトンポンプ阻害薬（PPI）で咳嗽が改善する場合，胃食道逆流による咳嗽と診断する．治療は，薬物（PPIやヒスタミンH2受容体拮抗薬など）による酸逆流抑制だけでなく，食事療法，生活習慣の改善，危険因子の除去を行う必要がある．治療は，2～3カ月は行ってみる必要がある．" @default.
