- W1982193885 abstract "Cognitive impairment and behavioral and psychological symptoms (BPSD) are commonly observed in the course of Alzheimer's disease. This was a 1-year prospective follow up study involving 187 consecutive patients diagnosed to have Alzheimer's disease according to NINCDS-ADRDA criteria. Patients were recruited from a specialist memory clinic in Hong Kong. Demographic data were collected. Folstein Mini-Mental Status Examination (MMSE), Clinical Dementia Rating (CDR), Alzheimer's disease assessment Scale - cognition (ADAS-cog) and Neuropsychiatric Inventory (NPI) were performed longitudinally for 1 year. 187 patients were recruited. The mean age was 77.73 (±7.51) years old. Mean education level was 3.30 (±4.47) years. 69% were female. 39.6% were APOEϵ4 positive (1 or 2 copies) . The mean one year change of NPI total score was 0.78. There were no significant relationship between the 1-year change in NPI score with age and education. Sex and the baseline ADAS-cog score were significantly associated with the 1 year change in NPI total score (p = 0.026 and p = 0.034). Both the baseline NPI total score (r =-0.574 p<0.000) and the individual score of the 9 domains were all negatively correlated with the 1 year change in the NPI total score. (r = -0.335 p <0.000; r = -0.165, p = 0.024; r = -0.46 p <0.000; r = -0.376 p <0.000; r = -0.263, p <0.000; r = -0.349, p <0.000; r = -0.393, p <0.000; r = -0.382, p <0.000; r = -0.295, p <0.000 for delusion; hallucination; agitation/aggression; dysphoria; anxiety; apathy; disinhibition; irritability; aberrant motor acts respectively) The change in NPI total score was also significantly correlated with declines in MMSE and CDR-SOB over 1 year. (r = -0.159, P = 0.03; r = 0.153, p = 0.038 respectively) Sex, baseline ADAS-cog and NPI total scores were independent predictors of the change in NPI total score over 1 year. Agitation and/or aggression and apathy were independent predictors for the 1 year change in NPI total score if individual domains entered into the model. Baseline global cognitive measure as well as neuropsychological measure was an important independent predictor of the decline in BPSD during the course of illness." @default.
