This method does not assess the technical and cognitive skills that truly reflect the expertise of the physician. Our objective is to measure the ability of the AccuTouch® Endoscopic Simulator to assess the competency of endoscopists at various levels of training and its utility as a training tool for fellows. Methods: 13 healthcare professionals participated in this study. They included 1st year fellows with no ERCP training (novice); intermediates with 2 years of ERCP training; experts with greater than 5 years and 1000 ERCPs, and video-game enthusiasts (VGE) with no endoscopic experience. Each completed 6 computer-generated cases using the endoscopic stimulator. The procedure time, identification of pathology, and rate of complications were assessed. The endoscopists also completed a questionnaire rating the realism and teaching value of the simulator. Results: The results for each group are shown in the table below. The expert and VGE group had the best procedure times. The VGE group had no perforations and a VGE participant scored highest on all parameters measured. The novice group was least likely to cannulate and identify pathology. The novice group had the longest average procedure time for any group which was only 17 minutes. The questionnaire had 20 questions rating the simulator on a numeric scale (1 = not realistic; 5 = very realistic). The simulator scored best on fluoroscopic imaging (4.2) but worst on realism of maneuvers for positioning (2.2). All agreed that the simulator was a useful teaching tool for fellows, although the endoscopists noted that advancing the scope and cannulation once in position were over-simplified. Conclusion: The ERCP simulator provides useful skills for training novice endoscopists learning ERCP. In its current format, the software is unable to simulate the difficulty of cannulation and finesse needed to distinguish an experienced endoscopist from a person adept at playing video games." @default.
